One of the most important things you can do to get ready for Graduate School is to complete your immunization requirements and upload needed documentation to the My Health Space portal.
Immunization Requirements
The University of South Carolina complies with federal health regulations requiring proof of immunity to Measles, Mumps and Rubella from all students. However, exemptions are allowed in specific circumstances. Refer to the Student Health and Well Being's Immunization page and frequently asked questions page for more information about required immunizations and exemptions.
New to USC students
Incoming graduate students who have never attended USC are required to complete the immunization process. Compliance can be achieved through three methods:
- Submitting immunization records to MyHealthSpace
- Receiving necessary immunizations at Student Health, or
- Obtaining a waiver by exemption from Student Health. Forms are available through MyHealthSpace under "Downloadable Forms." Upload buttons are provided on the forms page.
Failure to comply with immunization requirements will result in a registration hold that will remain until resolved.
Returning students who have not completed immunization requirements
Returning graduate students who have not completed immunizations requirements, but need to register for classes, may have the option to lift the hold by submitting an immunization hold release request form that will allow additional time to complete requirements. If approved, holds will be released the next business day after submission.
Failure to comply with immunization requirements will result in a registration hold that will remain until resolved. No additional holds will be lifted for returning students.
How do I submit proof of immunity?

How do I request a waiver exemption from Student Health?
You can find a Distance Learner Immunization Exemption Form, a Medical Condition Immunization Form and a Religious Immunization Exemption Form on MyHealthSpace under "Downloadable Forms." Upload buttons are provided on the forms page.
