• PhD transcript (unofficial or official)
• Any specific forms required by your licensure over site agency/employer
• Credit Card (for payment processing)
• Documentation of hours from field practicum

Please allow at least 15 business days from payment processing and all required materials beginning the day after submission of payment and all materials.

If the form is to go straight to my state board, will I receive notification?

Yes, you will be copied on any email submissions to designated oversite agencies.

If the form is to be mailed as a paper copy, will I receive notification?

Yes, you will receive an email confirmation.

Why do I have to provide documentation of my hours from field practicum?

The college does not maintain records of every student's practicum hours. If you do not have this documentation, we will confirm that you met the minimum requirements for the year(s) you were enrolled.
